FDVA invites interested Contractors to submit bids in accordance with this solicitation. The purpose of this solicitation is to establish an Agreement with FDVA for the Emory L. Bennett State Veterans’ Nursing Home located at 1920 Mason Ave Daytona Beach, Fl.32117 (Volusia County). The successful Contractor shall provide labor, services, equipment, tools, materials, and supplies required in the performance of the Agreement. Contractor shall remove the existing boiler #2 H.B. Smith Series 28A-8 CRN# D-605 Seria l# N92-1101 and drain water and reconnect piping. Contractor will install new boiler Lochinvar M# CHN2072-M13 Copper-fin gas boiler. Reconnect existing flue gas piping to new boiler. Wire electrical to the new boiler and replace existing pipe insulation. Open isolation valves and check for any leaks on new boiler. Startup of pump and boiler. Contractor will provide staff training, proper disposal of all project related waste, as well as one (1) annual system inspection, including but not limited to testing and repairs. The term of this Contract shall commence on the date of the Agreement’s full execution, with no renewals. FDVA requires specified services to be completed to the full satisfaction and acceptance of FDVA and any applicable authorities having jurisdiction, within one hundred and twenty (120) calendar days from the date of Agreement’s full execution.
