PERFORMANCE WORK STATEMENT (PWS)
Purpose. VHA Office of Emergency Management (OEM), will obtain a contract for calibration, maintenance, and repair services specifically for ten Berkeley Nucleonics Corporation (BNC) SAM 940 instruments owned by OEM/MERRT and required to support the VHA Medical Emergency Radiological Response Team (MERRT).
Background. The MERRT, as established by VHA Directive 0320.05, is a specialized team of healthcare professionals with radiological expertise that can be rapidly deployed to a VHA or off-site medical provider to render patient treatment and technical advice during a radiological emergency. The MERRT requires annual calibration, maintenance and servicing of radiation detection instruments utilized during field operations.
Objectives. The contract, once established, will provide a mechanism for MERRT members, who are geographically dispersed throughout the United States to have MERRT BNC 940 instruments calibrated and serviced annually.
Scope. The Contractor shall provide a service agreement for ten BNC 940 instruments.
Mandatory Tasks and Deliverables. The contractor shall provide a service contract on the ten BNC 940 instruments listed below to include annual calibration with a NIST traceable certification, all repair and service requirements, shipping charges and priority handling. The service agreements shall include system upgrades, firmware and software enhancements, battery replacement and unlimited application/technical support. If for any reason, any deliverable cannot be delivered on time according to the below schedule, the contractor shall provide a written explanation three days prior to the due date to the Point of Contact (POC). This written transmittal shall include a firm commitment of when the work shall be completed. This transmittal to the COR shall cite reasons for the delay, and the impact on the overall project. The COR will review collaboratively with the program office and facts and issue a response in accordance with the contract terms and conditions. Unless otherwise specified an electronic copy shall be placed in the designated CBO SharePoint site or other CBO-designated site. Place of Performance. The Contractor shall perform the work at the Manufacturer s facilities.
Type of Contract. A Firm- Fixed Price Contract
Security. The C&A requirements do not apply and a Security Accreditation Package is not required.
Service Performance Summary. Also called Performance Metrics or Performance Requirements Summary. This portion of the PWS explains to the vendor what the Government s expectations are, how (and how often) deliverables or services will be monitored and evaluated, and incentives that encourage the contractor to exceed the performance standards and that reduce payment or impose other negative incentives when the outputs/outcomes are below the performance standards. This may be an appendix to the PWS. Please note that every requirement in the PWS does not need to be listed in this document with a corresponding metric, only the most important ones. However, every metric in this document needs to be spelled out in the requirements.
The contractor shall perform repair, software and firmware updates and upgrades for the SAM 940 instruments specifically listed in the performance work statement. The contractor shall notify the POC if delivery of services will extend past the 15-day requirement.
