The performance period spans a twelve-month base term with two optional twelve-month extension years, with work primarily serving the Shiprock Service Unit and associated ambulatory care facilities in New Mexico, though the official place of performance is listed as Gallup, NM. Payment is contingent upon fully satisfactory service delivery, with no compensation for incomplete work, and performance will be formally assessed through a Contractor Performance Assessment Report used for future contracting decisions. The government mandates 100% compliance with standards of care, professional communication, and task completion as acceptance criteria, and surveillance will be conducted via periodic inspections and customer feedback. Although the contract is proposed as sole source due to the contractor’s prior performance history at the facility, any other firm may submit a capability statement via email by 10:00 a.m. MDT on July 17, 2026, to determine whether competition is warranted. The contracting office is located in Window Rock, AZ, and all official correspondence must be directed to Melissa Lake or Shannon Seaboy at the provided IHS email addresses.
