Issuing Procurement Unit: State of Utah Division of Purchasing
Conducting Procurement Unit: The Department of Health and Human Services
The purpose of this RFP is to enter into a contract to provide all-inclusive dental laboratory services at a per item cost and include all necessary equipment and supplies. This includes Traditional and Digital Impression Dentures.
It is anticipated that this RFP will result in a single contract award to a vendor that can offer both Traditional and Digital Impression Dentures to the responsive and responsible vendor with the highest score justified by the procurement code. The state has the option to award two separate contracts, one fore Traditional Impression Dentures and one for Digital Impression Dentures if needed.
Length of Contract: 5 years
Vendor must guarantee its pricing for one year.
