Mobile Integrated Healthcare Implementation Services

The Vermont Department of Health, under the Agency of Human Services, is awarding up to $5.37 million in federal funds through the Rural Health Transformation Program to implement regional Mobile Integrated Healthcare (MIH) programs aimed at improving care quality, reducing costs, and strengthening the rural healthcare workforce. This funding is exclusively for active service delivery and measurable patient impact, explicitly prohibiting initial planning or feasibility studies. Applicants must propose at least one core service bundle focused on Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Joint Replacement Care, each requiring defined patient contact schedules, in-home clinical assessments, medication reconciliation, discharge instruction reinforcement, disease-specific education, provider communication, and identification of social or environmental barriers. All selected applicants must be operationally ready or achieve readiness within 120 days of agreement execution, with services to be delivered from August 1, 2026, through September 30, 2027. Geographic preference is given to entities serving rural Vermont counties outside Chittenden County, though Chittenden County applicants may be considered based on their rural service areas. Only registered businesses with a Vermont Contractor’s Business Account Number, an EIN, a signed W-9, and compliant insurance certificates are eligible. Payment is contingent upon submission of error-free invoices within 60 days of the grant period’s end, meeting or exceeding approved performance metrics including reduced ED visits and hospital readmissions, and passing quarterly and monthly reporting requirements. Invoices must be submitted electronically, and all expenditures require three years of documentation retention subject to annual audits. Strict compliance with HIPAA safeguards, drug-free workplace policies, non-discrimination laws, OSHA guidelines, and federal lobbying restrictions is mandatory, with all program oversight coordinated through a designated MIH Medical Director responsible for quality assurance. Letters of Interest, including a completed provider cost questionnaire and high-level budget, must be submitted electronically to Danielle Tucker by July 10, 2026.
