The City of Georgetown, Kentucky (the “City”) is soliciting proposals for a Near-Site Direct Primary Care Facility for its employee health plan members. Primary care medical services will be at the cost of the City and free to employees, utilizing a membership-based concept. All addenda and Q&A posted in the “Public Purchase” portal, if any, shall be considered in preparing a proposal, and such addenda shall be made a part of this RFP. Before submitting a proposal, it is incumbent upon each proposer to be informed as to whether any addenda have been issued, and the failure to cover in the proposal any such addenda may result in disqualification of that proposal. No interpretation of the meaning of the plans, specifications or other pre-proposal documents will be made to any proposer orally. Every request for such interpretation should be submitted through the “Public Purchase” portal linked here: http://www.publicpurchase.com/. Any and all such interpretations and any supplemental instructions, if issued, will be posted to the “Public Purchase” portal. Failure of any proposer to receive any such supplementation or interpretation shall not relieve such proposer from any obligation under their proposal as submitted. All posts so issued shall become part of the RFP and/or contract documents. All questions related to registration for and use of the “Public Purchase” portal shall be directed to Stacey Clark at stacey.clark@georgetownky.gov.
