Health Promotion & Disease Prevention (HPDP) Wellness Program Coordination Services


Northern Navajo Medical Center (NNMC), Shiprock, New Mexico


The Indian Health Service (IHS), Shiprock Service Unit is issuing this Sources Sought Notice (SSN) to identify capable Indian-owned economic enterprises — large (IEE) or small (ISBEE) — that can provide professional wellness coordination and health promotion services in support of the Northern Navajo Medical Center (NNMC) Health Promotion and Disease Prevention (HPDP) program. Services include wellness facility access for referred IHS patients and community beneficiaries, community-based health promotion event support, mental health wellness initiative support, health education training, culturally appropriate nutrition and cooking programs, concussion management equipment training support, and crisis prevention and de-escalation training for school staff within the Shiprock Service Unit.


Other capable firms are also invited to respond to this SSN. The Contracting Officer will consider responses for Buy Indian Act (25 U.S.C. 47) opportunities first, then for set aside programs under the Small Business Act (41 U.S.C. 3104).


Background and Description of Requirement


The Shiprock Service Unit of the Indian Health Service serves a large and geographically dispersed Navajo population across 20 outlying chapters. Access to health promotion and wellness activities is limited for residents of these communities. The IHS HPDP department is seeking a qualified contractor to deliver a comprehensive, community-based wellness program that:


  • Provides IHS patients and community beneficiaries — who present with a written referral or written provider guidance from an NNMC-authorized provider — access to exercise equipment and aquatic wellness facilities at contractor-secured locations within 25 miles of NNMC Shiprock, New Mexico;
  • Plans, coordinates, and executes no fewer than eight community HPDP promotional events per performance year;
  • Supports mental health wellness initiatives in a non-clinical capacity;
  • Provides health education training and supplies for students, parents, and community members;
  • Delivers a structured, culturally grounded community wellness program engaging a minimum of 50 unduplicated youth participants per performance year;
  • Develops and delivers a healthy living and cooking education program no fewer than twice per month, focused on heart-healthy and diabetes-friendly Native American cuisine;
  • Supports Impact Concussion equipment training for local schools within the Shiprock Service Unit;
  • Provides evidence-based crisis prevention and de-escalation training for school staff.

All services shall be performed at contractor-secured or contractor-managed facilities located within 25 miles of NNMC Shiprock, New Mexico. The contractor is responsible for securing all necessary facility agreements, staffing, supplies, and equipment. This requirement does not involve physical therapy, medical diagnosis, clinical treatment, or any service requiring licensure as a healthcare provider.


The contemplated contract is a firm-fixed-price (FFP) contract with a base year and three one-year option periods (four years total).

3. Indian-Owned Economic Enterprise (IEE/ISBEE) Documentation:


Indian-owned Small Business Economic Enterprises (ISBEEs) and Indian-owned Economic Enterprises (IEEs) must also submit a completed IHS IEE Representation Form (attached) in accordance with HHSAR 326.606-1(b).
