Notice of Intent to Award – Blanket Purchase Agreement


The Navajo Area Indian Health Service, Chinle Service Unit, Chinle Comprehensive Health Care Facility (CCHCF) in Chinle, Arizona intends to place a Blanket Purchase Agreement (BPA) with a Indian Small Business Economic Enterprise (ISBEE) vendor, National Environmental, INC; DBA: Buy Indian Medical for the Freestyle LIBRE Glucose Monitors on an as-needed basis.


The anticipated period of performance is for one year. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 325415 – In-Vitro Diagnostic Substance Manufacturing with a small business size standard of $16.5.


No further awards will be made unless the need for the population health department at the Chinle Service Unit, Chinle Comprehensive Health Care Facility changes. The prospective, award BPA as identified in this notice are sufficient at this time.
