Nursing Home Therapy Services

Background The County of LaSalle, Illinois, is seeking qualified firms to provide Therapy Services at the LaSalle County Nursing Home. The nursing home operates as a skilled nursing facility with 79 beds, offering both post-acute and long-term care. The goal of this contract is to ensure comprehensive therapy services including physical, occupational, and speech therapy for residents. Work Details The awarded contractor will provide the following services: - Conduct therapy evaluations within 48 hours of receiving a physician's order. - Provide treatment in accordance with the physician's plan of care. - Maintain accurate medical records within 24 hours of service completion. - Offer onsite evaluation and treatment services up to six days a week. - Ensure all therapists possess appropriate Illinois/national licensure/certification. - Comply with Medicare Resource Utilization Groups (RUGs) and Patient Driven Payment Model (PDPM) regulations. Additionally, the contractor will participate in interdisciplinary patient care conferences, conduct quarterly quality assurance audits, and maintain confidentiality of patient records. Period of Performance The contract will be for one year with the option to renew for four additional one-year terms, totaling a maximum of five years. Bidder Requirements Bidders must comply with all applicable Federal and State laws including those from the Illinois Department of Public Health (IDPH) and Centers for Medicare & Medicaid Services (CMS). Contractors must have necessary qualifications and certifications to provide required services and must not be excluded from any State or Federal health care program. Proof of insurance coverage must be provided within fifteen days after contract award.
