The objective of Rural Health Transformation Community Health Worker (CHW) Training and Technical Assistance is to ensure Ohio has a well-prepared and sustainable Community Health Worker workforce equipped to meet the unique needs of rural communities. The Ohio Department of Health aims to contract with a vendor who is qualified to develop and deliver targeted skill-building and professional development opportunities that will strengthen CHWs’ capacity to connect rural Ohioans to essential resources and improve health outcomes.

Each Offeror must submit a technical proposal and cost proposal as part of its total proposal before the closing date and time, which is 06/15/2026 at 3:00PM.
