The Air Force Life Cycle management Center (AFLCMC) Human System Division (AFLCMC/WNU), at Wright Patterson Air Force Base has a requirement for Periodic/Preventative Maintenance on Vibration Table Assembly for the United States Air Force (USAF) - FA8606-M-17-0017. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is a request for a quote (RFQ) The NAICS code is 811219. The contractor shall provide all personnel, labor, transportation, tools, technical data/manuals, expertise, supervision, and all services necessary to perform the annual preventative maintenance on the vibration table assembly for the following locations: 2060 Monahan Way, Building 17, Wright Patterson Air Force Base, Ohio 45433. The contractor shall provide annual preventative maintenance to the above mentioned vibration table assembly, which consists of the shaker (model # LS437A), slip table (model # GT1440M), power amplifier (model # MPA409), and blower (model # HP4). Please see the PWS for further information. Offerors may use another format for submission of a response to this RFQ. All responses must contain the minimum information required as listed below.
