PHPA 4124: Statewide Care Coordination Program for Children and Youth with Specific Health Care Needs

The Maryland Department of Health, Office of Children and Youth with Specific Health Care Needs, is seeking a single entity to provide consistent statewide care coordination services for children and youth aged 0 to 22 and their families. This cost-reimbursement grant aims to ensure equitable access to medical and community services through a family-centered, tiered care delivery model. The selected provider will be responsible for operationalizing a four-tier service model, providing capacity-building training for caregivers and youth, and utilizing standardized tools such as the Care Coordination Intake and Tier Determination forms. Performance is measured through monthly check-ins, quarterly progress reports, and annual final reports, with a required Master Work Plan due within 10 business days of project kickoff. The award will be granted to the applicant deemed most advantageous to the State, with technical and financial factors weighted equally. Technical evaluations focus on the quality of the implementation plan, staffing strategy, and program assessment framework, while financial proposals are assessed based on cost reasonableness.
