Combined Synopsis/Solicitation IAW 12.603 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.603 - Streamlined solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation proposals are being requested and a written solicitation will not be issued. (ii) Request for Quotation number 16-247-SOL-00026. (iii) This Request for Quotation solicitation document and incorporates provisions and clauses are those in effect through the most current Federal Acquisition Circular. (iv) The North American Industry Classification System (NAICS) code is 811219 and the business size standard is $20.5 million. (v) All quotes shall be in accordance with FAR 52.247-34, FOB Destination (vi) The Phoenix Area Indian Health Service, for the Phoenix Indian Medical Center, is requesting quotes for the Preventative Maintenance and Repair Services to include all labor, materials, equipment, supplies, permits and licenses on inspection and repairs on Hydrogen Peroxide Sterilizer equipment. (vii) Place of Performance is at the Phoenix Indian Medical Center, 4212 North 16th Street, Phoenix, Arizona 85016, period of performance is February 15, 2016 to February 14, 2017, and this location is where delivery and acceptance of requested services. This requirement is for a base year with two (1) year options. (viii) The following FAR Clause provisions apply to this solicitation; FAR 52.212-1 - Instructions to Offerors - Commercial Items (Oct 2015) (ix) The following FAR Clause provisions apply to this solicitation; 52.212-2 - Evaluation - Commercial Items (Oct 2014). The Government intends to award a firm-fixed price purchase order resulting from this solicitation. Award will be made to the best value to the Government.
