Provider Agreement (PA) - General Relief Assisted Living Home

The State of Alaska Department of Health, Division of Senior and Disabilities Services, is establishing Provider Agreements with qualified Assisted Living Home Providers to deliver services to eligible adult Alaska residents under the General Relief Assisted Living Home Provider Agreement program. These agreements are governed by Alaska Administrative Code 7 AAC 81, 7 AAC 47, and 7 AAC 75, as well as Alaska Statutes AS 47.24 and AS 47.33. Providers must maintain all necessary licenses, certifications, and insurance, as failure to do so results in immediate termination. The scope of work includes maintaining detailed client records, implementing specific Assisted Living Plans of Care, and coordinating with Department representatives and case managers. Facilities must adhere to strict safety and environmental standards, including ADA compliance, fire codes, and specific temperature requirements of 68 degrees Fahrenheit for adult care. Administrative requirements include strict medication labeling and storage protocols, with controlled substances requiring locked, permanently affixed containers. Providers must also comply with federal mandates, including the Civil Rights Act of 1964, the Drug Free Workplace Act of 1988, and the Americans with Disabilities Act of 1990. Financial reimbursement is managed through warrants, and specific augmented rates are available for temporary placements, transitional services, and Category III residents. The selection process allows the department to refer recipients to providers or allow recipients to choose their own, provided the chosen entity enters into a formal agreement. Performance is monitored through on-site inspections by department representatives to ensure quality standards are met.
