Reference Far 13.303. The solicitation number is W912L5-12-T-0007 and is issued as a RFQ. This announcement constitutes the only solicitation; proposals are being requested and written solicitations will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-34. The associated North American Industrial Classification system (Naics) code is 621511, size standard 13.5 mil. This requirement will be a five year non-binding agreement for either party. The Government is obligated only to the extent of authorized services actually made under the (BPA).Termination will require a 30 day written notice by either party. Price change will require a 30 day notice due to inflation cost. This requirement supports the Delaware National Guard for Standard Occupational Medical Exams: pre-placement, baseline, job-transfer, and termination exams. It also supports the Medical Surveillance Exams Program. Physical exams and testing will be provided in accordance with the Occupational Safety and Health Administration (OSHA) and other Department of Defense Regulations. It is understood that both programs are to be treated separately, invoiced separately and may be more than one contractor providing the designated services. Requirements and Guidelines will be listed in the attached Performance Statement of Work. The Delaware Occupational Safety Health Nurse Manager will be the assigned Contracting Officer Representative (COR) to complete the Quality Assurance Weekly reviews. Each individual service will not exceed $2,500 per call. Payment will be made with the Government Purchase card by an authorized cardholder within the Delaware Army National Guard. Each invoice will list a BPA agreement number, call number, date of service, company name, itemized list of services furnished, unit price, quantity, less any discounts and individual pricing per service. Monthly pricing may be authorized through Wide Area Workflow Paperless invoicing, (for large groups of soldiers receiving service within the same day). The attached DENG Medical Surveillance and the DENG Standard Occupational Health price listing is to be completed and returned by interested parties by due date, if to be considered. All medical results and documentation shall be provided within the time-lines stated within the Performance Statement of Work, and reviewed by the Occupational Health and Safety Nurse (OHN) manager, for the Delaware Army National Guard. Examinations and testing locations are to be within New Castle, Kent, Sussex County, Delaware, Medical Lab Facility. Average number of patients per month (4 or 5). Calls will be placed through the DENG Occupational Health Nurse and and point of contact assigned from the Human Resource Department. Any state employees requiring testing (firefighters, security police, etc.) will be announced at time of appointment and invoicing will be separate. Past performance, qualifications and reference are required up to 3 years. The government is non liable for medical indemnity. Best Value is to be considered.
