The contract involves providing comprehensive home-based and telephone follow-up services for participants of the Baltimore Longitudinal Study on Aging (BLSA) who are unable to visit the study clinic in Baltimore. The selected contractor will be responsible for updating participants’ contact information, establishing their status and willingness to participate in either home visits or telephone interviews, and then conducting detailed health assessments or interviews accordingly. The health information collected will cover multiple outcomes including dementia, mobility limitations, disability, frailty, coronary heart disease, cancer, and more. For home visit participants, the contractor will also obtain blood samples for clinical chemistry tests, DNA extraction, and storage of biological materials. Additionally, the contractor must provide participants and their families with study information and obtain informed consent for participation and the use of previously collected biological specimens. All collected data will need to be compiled into a database fully compatible with the BLSA core database, with guidance provided by the Project Officer. This contract is expected to be a five-year, cost-reimbursement type agreement, with solicitation issued in December 2006 and the award anticipated around June 2007. Proposals from all responsible sources will be considered, and the contractor will furnish all necessary personnel, materials, equipment, and facilities not provided by the government to fulfill the contract requirements.

The National Institute on Aging (NIA), National Institutes of Health (NIH), has a requirement to perform home-based visits and, alternatively, telephone visits to Baltimore Longitudinal Study on Aging (BLSA) participants who, for different reasons, cannot come to the study clinic in Baltimore for a traditional follow-up evaluation. The successful offeror will be required to furnish all the necessary services, personnel, material, equipment and facilities, not otherwise provided by the Government, needed to perform the following work: (1) Update contact information on BLSA participants who are not currently coming to Baltimore for follow-up assessments over the last three years and, based on current information, are not deceased; (2) Make telephone contact with the BLSA participants potentially eligible for home visits or telephone interviews to establish their status, their willingness to participate in the home visit program or, if not, in a telephone interview; (3) In BLSA participants enrolled in the home visit program, conduct an extensive ascertainment of their health status, aimed at ascertaining multiple health-related outcomes (dementia, mobility limitation, ADL disability, frailty, CHD, cancer etc.); (4) In BLSA participants who are only willing to participate in a telephone interview, collect standard information through a telephone interview; (5) Collect a blood sample to be used for basic tests of clinical chemistry, DNA extraction and storage of cells, serum and plasma; (6) Provide participants and their families with a detailed description of the study, and obtain informed consent for participation, including consent for the use of previously collected biological specimens; and (7) Provide to the NIA a database with all the tracking information and all information collected over the telephone interview and during in person home visits, maintaining full compatibility with the BLSA core database. Information on the core database will be provided by the Project Officer. It is anticipated that a five year, cost-reimbursement type contract will be awarded, with award made on or about June 1, 2007. The solicitation will be issued on or about December 6, 2006 and will be posted on this Web site. Offerors are responsible for routinely checking this Web site for any amendments to the solicitation. Proposals will be due approximately 60 days after the date of issuance of the solicitation. All responsible sources may submit a proposal, which will be considered by the Government. Solicitation # 27-0904004-004
The South Dakota Department of Health, Office of Home Visiting, is soliciting proposals for partner agencies to provide staffing and service delivery for the Bright Start program. This initiative utilizes federal Maternal, Infant, and Early Childhood Home Visiting funds to deliver evidence-based home visiting services to eligible pregnant women across South Dakota. While the program currently employs the Nurse-Family Partnership model, selected agencies must be capable of transitioning to other evidence-based models if federal guidance or funding requirements change. The contract is intended for a negotiated term of five years, with an anticipated start date of January 1, 2027. Selected agencies will be responsible for the hiring, supervision, and personnel management of key staff, including a Team Lead, Nurse Home Visitors, a Community Health Worker, Program Assistants/Referral Specialists, and a Continuous Quality Improvement Specialist. Performance will be measured through clinical nursing standards, chart audits, and data integrity checks, with the state providing supporting documentation for evaluations. Agencies must maintain comprehensive insurance coverage, including one million dollars each for commercial general liability, professional liability, and business automobile liability, and must comply with South Dakota law regarding workers compensation. Proposals are due by November 20, 2026, and must be submitted electronically to Carrie Churchill. The evaluation process is a competitive negotiation based on technical competence, available resources, past performance, familiarity with the project locale, and project management techniques. Cost proposals will be evaluated independently using a specific budget template, with salary caps in place for Nurse Home Visitors and Nurse Team Leads based on career band market values.
