The Department of Veterans Affairs, Southeast Louisiana Veterans Healthcare System, required Community Nursing Home (CNH) services for veterans in the Destrehan, St. Charles Parish, Louisiana catchment area. The objective was to establish a Fixed Unit Price Indefinite Delivery Contract (IDC) for a five-year period of performance to provide appropriate levels of nursing care. To be eligible, providers were required to be licensed as a Nursing Home by the State of Louisiana and certified as a Medicaid facility. Following a market survey and sources sought process under NAICS 623110, the contract was awarded as a sole source to the Community Care Center of Destrehan, LLC, doing business as Ormond Nursing and Care Center. This decision was justified under FAR 6.302-1 and FAR 13.5, as the vendor was identified as the only source in the parish capable of providing the necessary services without causing undue hardship to veterans and their families through relocation. The resulting contract, numbered 36C25621D0087, has an estimated total value of 4,955,970.00 dollars and covers the performance period from October 1, 2021, through September 30, 2026.
