Full Description

This is a combined synopsis/solicitation for commercial non-personal services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This action is a 100% small business set-aside. The applicable North American Industry Classification System (NAICS) code is 624190 with a size standard of $7 million. Payment will be made by the Defense Financial and Accounting System (DFAS); therefore, you must be registered in the Central Contractor Registration (CCR) (http://www.ccr.gov/), as well as, Wide Area Work Flow (WAWF) (https://wawf.eb.mil/). Award shall be made to the most technically acceptable, best value proposal. You shall submit commercially available literature that demonstrates you are able to meet all of the requirements specified in the attached Statement of Work. From the Federal Acquisition Regulation (FAR), the following provisions and clauses are applicable to this solicitation: FAR 52.212-1, 52.212-3, 52.212-4, and 52.212-5 with applicable items - 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33, 52.222-41, 52.222-42, 52.222-43, 52.222-50, 52.237-11 and 52.237-7. From the defense Federal Acquisition Regulation Supplement (DFARS), the following provisions and clauses are applicable to this solicitation: DFARS 252.212-7000, and 52.212-7001 with applicable item - 252.232-7003. This requirement is a non-personal health care services contract as defined in FAR 37.101, under which the contractor is an independent contractor for a Licensed Social Worker at White Sands Missile Range (WSMR), NM. The contract will be for one base year plus three option years for the services specified under the attached Statement of Work. The Government will provide office space, computer, and office supplies. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment). The contractor shall maintain proof of Professional Liability Insurance with a minimum of $1,000,000.00. Please use the attached Proposal Summary as a template for your priced proposal. Proposals will be approved and interviews will be scheduled ONLY if the contractor meets all of the minimum requirements specified in the Statement of Work.
