THE ACTION DESCRIBED HEREIN IS A MODIFICATION TO AN EXISTING CONTRACT, DOLJ06QA00004. THIS OPPORTUNITY WAS AVAILABLE ONLY TO THE CONTRACTOR IDENTIFIED BELOW. This opportunity was available only to Res-Care, Inc. under contract number DOLJ06QA00004.
