RFGA2027-004 Metabolic Formula Program

The Arizona Department of Health Services, through the PHS Prevention agency, is soliciting applications for RFGA2027-004 to establish a statewide network providing prescribed metabolic formulas. This program serves children, youth, and adults with metabolic disorders who are uninsured or underinsured and lack other means of covering the cost of these specialized dietary manipulations. The grant provides estimated annual funding of 175,000 dollars on a calendar-year basis. Awardees must procure and provide only the approved formulas listed in the contract's specifications and are required to submit monthly contractor expenditure reports and distribution reports to the state. Applications must be submitted electronically via the eCivis platform by October 29, 2026. Evaluation is based on a weighted scoring system, with 70 points allocated to narrative responses and 30 points to the budget workbook. The state reserves the right to issue partial awards or select any application that serves the best interest of Arizona. Compliance requirements are extensive, including adherence to 2 CFR 200, HIPAA regulations, and various civil rights acts. Additionally, grantees must comply with state executive orders regarding non-discrimination and immigration laws, and maintain insurance where the contractor's coverage is primary and the state's is excess.
