The GLAC (Great Lakes Acquisition Center) is soliciting proposals to provide all labor, material, tools, and equipment for the removal and proper disposal of the general waste, Regulated Medical Waste (RMW), Pathological and Chemotherapeutic Waste, Re-usable Sharps Containers as well as recycling of mixed grade paper and loose cardboard as further defined in the RFP. Services will be required for the period beginning on or about April 1, 2008 through September 30, 2008 plus four (4) Option Renewal Periods. Services shall be provided at the following locations in VISN 12: the Jesse Brown VAMC, the Lakeside CBOC, and the Auburn Gresham Clinic in Chicago IL plus the Adam Benjamin Jr. CBOC Located in Crown Pt. IN. This RFP is being issued in accordance with 38 U.S.C. 8127, for the Department of Veterans Affairs and PL 109-461 and is 100 % Set-Aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). No telephone calls will be accepted. Contract type is firm-fixed price and the NACIS Code is 562112 with a size standard of $11.5 Million.
