This is a combined synopsis/solicitation for commercial items prepared in accordance
with the format in RFO FAR Subpart 12.2, as supplemented with additional
information included in this notice. This announcement constitutes the only
solicitation; quotes are being requested and a written solicitation will not be issued.


The Defense Health Agency Contracting Activity Healthcare Contracting Division -
West, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award
a Blanket Purchase Agreement (BPA) with Firm Fixed-Prices for Scleral Contact Lenses for Womack Army Medical Center (WAMC), Ft. Bragg, NC.


The solicitation number for this request for quotation (RFQ) is HT940626QE090. The
solicitation document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 2026-01. The Standard Industrial Classification
