SAMPLING:
THE SAMPLING METHOD SHALL BE IN ACCORDANCE WITH MIL-STD-1916 OR ASQ
H1331, TABLE 1 OR A COMPARABLE ZERO BASED SAMPLING PLAN UNLESS OTHERWISE
SPECIFIED BY THE CONTRACT. IF THE APPLICABLE DRAWING, SPECIFICATION,
STANDARD, OR QUALITY ASSURANCE PROVISION (QAP) SPECIFIES CRITICAL, MAJOR
AND/OR MINOR ATTRIBUTES, THEY SHALL BE ASSIGNED VERIFICAITON LEVELS OF
VII, IV AND II OR AQLS OF 0.1, 1.0 AND 4.0 RESPECTIVELY. UNSPECIFIED
ATTRIBUTES SHALL BE CONSIDERED AS MAJOR UNLESS SAMPLING PLANS ARE
SPECIFIED IN APPLICABLE DOCUMENTS. FOR MIL-STD-1916, THE MANUFACTURER
MAY USE THE ATTRIBUTE OR VARIABLE INSPECTION METHOD AT THEIR OPTION OR
PER THE CONTRACT. MIL-STD-105/ASQ Z1.4 MAY BE USED TO SET SAMPLE LOT
SIZE, BUT ACCEPTANCE WOULD BE ZERO NON-CONFORMANCES IN THE SAMPLE LOT
UNLESS OTHERWISE SPECIFIED IN THE CONTRACT.

This item is procured in accordance with a United States Department of Agriculture, Forest Service specification. These Specifications are located at: https://www.fs.usda.gov/t-d/programs/fire/specs.htm
Fire Shelter The fire shelter shall be a Large size shelter in accordance with (IAW) Forest Service Specification 5100-606E dated September 1, 2023, and drawing MTDC-1000G dated August 25, 2023. The fire shelter cloth shall be IAW Forest Service Specification 5100-607F dated September 1, 2023. The polyvinyl bag shall be IAW Forest Service Specification 5100-608D, dated February 11, 2020, drawing MTDC 1001F dated August 25, 2023, and drawing MTDC 1004E dated August 25, 2023. (Note: Forest Service Protocol for Evaluating Off-Gas Toxicity of Fire Shelter Material, 5100-615 is in Revision B dated September 1, 2023.)
Case The case shall be in accordance with Forest Service Specification 5100-609B dated September 1, 2023; and MTDC Drawing 1002F dated August 25, 2023.
Liner The liner shall be in accordance with Forest Service Specification 5100-610B dated September 1, 2023; and MTDC Drawing 1003D dated August 25, 2023.
First Article required IAW 5100-606 para 3.1 and 6.3 First article sample (3) are required. First Article samples shall be sent to:Missoula Technology and Development Center (MTDC) 5785 Hwy 10 W Missoula, MT 59808 Phone: 406-329-3900 Fax: 406-329-3719 FAT samplepackage requirements: Exterior of the package should clearly/boldly identify "FAT SAMPLE NSN xxxx-xx-xxx-xxxx" Address label shouldinclude the following line: ATTN: Specifications & Standards Project Leader (First Article Test Items) All supporting documents (e.g. certificate of conformance) should be included within the FAT sample package.
Certification. The contractor shall provide the following information on certificates of conformance: Where certificates of compliance are submitted, the Government reserves the right to check test such items to determine the validity of the certification (see 4.4.3). Unless otherwise specified (see 6.2), in addition to the certificates of compliance required for each component as specified in their respective specifications. 5100-607 Cloths, Laminated, Fire Shelter, M-2002. 5100-608 Polyvinyl Chloride Bag, Fire Shelter, M-2002. 5100-609 Case, Carrying, Fire Shelter, M-2002. 5100-610, Liner, Carrying Case, Fire Shelter, M-2002. The contractor shall provide certificates of compliance, along with test records where applicable, for the following: Thread, quartz (3.3.2) Thread, nylon (3.3.3) Thread, Kevlar (3.3.4) Thread, glass fiber (3.3.5) Cloth, Kevlar (3.3.6) Tape, glass fiber (3.3.7) Webbing, nylon, 2 inch (3.3.8) Tape, nylon, 1 Inch (3.3.9) Shake handle insert (3.3.10) Label, sewn in (3.3.11.1) Fastener tape (3.3.13) No thread lubrication other than as specified in 3.3.2.1 (3.4.2.5) Certificates shall include the following: Specification, type, class, form, etc. as applicable, Quantity purchased, Purchase source, address, and telephone number Purchase date Lot number traceable to materials used in production Contract number. Quality conformance inspection IAW 5100-606 para 4.4
NDIVIDUAL COMPONENT MARKING: Markings on each component of the complete item shall be as specified by the specifications for each component, as modified by this IPD. Note that each component has its own NSN and name, which differs from the NSN and name for this IPD
This contract will be administered by DCMA and the source inspection with be conducted by the assigned QAR.

MARKING: Shipments to the Department of Defense (DOD) shall be marked in accordance with MIL-STD-129.
