The Department of Veterans Affairs, Portland VA Health Care System, intends to award a base plus four (4) option years firm-fixed price for DISC Assessment Tool that includes signature client Toolbox in order to provide development opportunities.

INSTRUCTIONS TO RESPOND TO SOURCES SOUGHT SYNOPSIS:
Any vendor interested and capable of meeting this requirement should provide a written statement to the Contracting Officer. Vendor response must include at a minimum:
