Veteran-Directed Care Assessment and Options Counseling Services

The Santee-Lynches Regional Council of Governments, through the Santee-Lynches Area Agency on Aging, is soliciting proposals for Veteran-Directed Care (VDC) assessment and options counseling services. The selected contractor will provide participant-centered services, including intake, enrollment support, and Veteran-centered assessments of strengths and needs to help eligible Veterans remain safely in their homes and communities. Key responsibilities include providing options counseling regarding budgets and caregivers, managing employer and employee enrollment packets for payroll processing, and conducting home visits within the Santee-Lynches service area in South Carolina. The contract is anticipated to be executed on December 30, 2026, with a service start date of January 1, 2027. Proposals must be submitted by October 30, 2026, and will be evaluated using a 100-point weighted system. Compensation is based on a negotiated methodology, which may include per-completed assessment, per-enrollment packet, per-Veteran monthly support, or hourly rates. Contractors must be registered with Sam.gov, comply with HIPAA and 45 C.F.R. privacy requirements, and maintain all supporting documentation for at least ten years. Additionally, the provider must adhere to ADA standards, Title VI of the Civil Rights Act, and all applicable federal, state, and local safety and health regulations.
