COMBINED SYNOPSIS/SOLICITATION Subject: Veterinary and Dental Medical Supplies This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number HHS 12-100-SOL-00020 is issued as a Request for Quotation (RFQ). This procurement is being conducted under the simplified acquisition procedures in FAR Part 13. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-57. This RFQ is issued as a small business set aside under NAICS code 423450. The size standard of 100 employees. The Government intends to award a single indefinite delivery, indefinite quantity purchase order. The guaranteed minimum is $5,000. The ceiling is $150,000. The following contract line item number (CLIN) is included in this RFQ: These Items are to be ordered in accordance with the statement of work. SEE ATTACHED Schedule of Services and Supplies Pricing: Vendors shall provide Firm Fixed Price (FFP) quotes. Statement of Work Background ASPR provides emergency medical care to the victims of public health emergencies and catastrophic events that overwhelm the capacity of state and local emergency medical systems to respond to these disasters. ASPR currently has teams located across the nation that are prepared to deploy within hours to respond to an all hazards environment. ASPR has positioned pharmaceutical caches/kits at various locations around the Continental United States, Alaska, Puerto Rico, and Hawaii. These caches/kits of pharmaceuticals must be in a constant state of readiness for immediate use to meet operational requirements. Medical caches for National Veterinarian Response Teams are located within the warehouses of ASPR. Purpose The purpose of this contract is to establish a contract that allows The Department of Health and Human Services (DHHS)/ASPR/OPEO to readily replenish its supplies as needed when responding to disasters (during response) and when in steady state. During a disaster, ASPR requires additional dental and veterinary supplies for purposes of re-supplying caches/kits. The exact, dental, and/ or veterinary supplies, quantities and delivery requirements are not known in advance and may vary considerably for re-supply. The resulting contract provides for preparedness operations field re-supply of, dental, and/or veterinary supplies in the disaster-affected regions. See attached 1.
