Full Description

NOTICE OF INTENT TO SOLE SOURCE REQUIREMENT The Department of Veterans Affairs, VA Healthcare System, VISN 10, Contracting Activity intends to negotiate a sole source contract under FAR Part 13.106-1(b) -Soliciting from a Single Source - Only One Reasonable Source (Brand Name). The vendor will be Zoll Services LLC. The associated NAICS code is 334510. The business size is 1250 employees. The anticipated award date is 10/15/2018. A firm fixed price contract is anticipated and shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate with only this source. This notice of intent is not a request for competitive quotes or proposals. It is not a solicitation or request for offers. No solicitation package is available. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Responses shall include: (1) Business Name and Address, (2) FSS Contract Number, (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED etc. Responses shall be submitted to John Saunders, Contracting Officer, at contact@example.gov no-later-than Friday, October 12, 2018 at 4:00pm EST. The Contractor shall provide the deliverables outlined below to the VAMC Indianapolis located at 1481 West 10th Street Indianapolis, IN 46202. DELIVERABLES: The vendor shall supply Zoll Life Vest, Wearable Defibrillators (AED GARMET w/EELECT, Code K0606), that will protect patients at risk of SCA when a patient's condition is changing and permanent sudden cardiac death risk has not been established. These defibrillators require no bystander intervention, have a 98% first shock success rate, allow for 92% of patients to stay at home or arrive conscious to the ERA during an event. Period of Performance: 10/15/2015 through 07/14/2019
