The State of New York has selected Public Health Solutions (PHS) as the Social Care Network (SCN) lead entity for the boroughs of Manhattan, Queens, and Brooklyn in connection with the New York State (NYS) Department of Health, Office of Health Insurance Programs’ RFA #20417 entitled “New York Health Equity Reform: Social Care Networks”. As a SCN lead entity, PHS will create and maintain a network of community-based organizations (CBOs) that will screen, navigate, and deliver social care services to eligible Medicaid members to address their health-related social needs (HRSN). The WholeYouNYC SCN will be a committee of PHS reporting directly to PHS’ Board of Directors and chaired by one or more PHS Board Members, with staffing support provided by NYC SCN leadership. The committee will be comprised of at least 51% regionally-representative Community-Based Organizations (CBOs) alongside multiple stakeholders e.g., Community Advocates, Medicaid Members, Healthcare and Care Management Providers (Behavioral Health Providers, FQHCs, Health Homes; Health Systems), Mental Health and Substance Use Disorder Providers. New York State expects SCN lead entities to ensure their governing bodies include representation from a multitude of individuals across race, ethnicity, disability, age, and socioeconomic status. The SCN Committee will be responsible for setting strategic goals and supporting operational and program decision-making, across the network.
