The following project will be accomplished as Washington State Small Works Roster project in accordance with RCW 39.04.155. 2. The State of Washington Prevailing Wage Rates applicable for this public works project, which is located in Clark County, may be found at the following website address of the Department of Labor and Industries: Prevailing Wage Rates. 3. The contractor shall pay the prevailing wage rate per the day of when the work was performed according to the trade performed. All bidders must be on the MRSC Small Works Roster (www.mrscrosters.org) in order to submit a quote. Scope of Work The garage door in Bay #18 of our North Maintenance Building (M3) was struck by a vehicle. One lower panel of glass is broken (see attached pictures), and the frame of door appears to be bowed outwards. The right side of the door’s tracks have been sheared from the metal framing holding the door into place. It has approximately 1-1.5” of movement front to back. The left side of door is secured to the door frame with no visible damage. The door is currently in the closed position and locked out to reduce safety concerns if it was to be operated. We do not know the operational status of the door. Contractor will provide labor, materials, equipment, and supervision necessary to inspect, repair, and restore the glass (89" H x 46" W) bi-fold door, rollers, track, and glass panel to proper safe operation of an existing vertical garage door system. All work shall be performed in accordance with manufacturer specifications, applicable codes, and site safety requirements. The materials will be new, manufacturer-approved or equivalent replacement parts. All parts used shall be suitable for the door type, size, and usage. Any additional parts outside original scope must be approved prior to installation by C-TRAN's Contract Manager covering this project. Contractor shall conduct a site inspection to determine the level of damage to the door, tracks, and associated hardware indicating the need for replacement to return the door to safe, fully operational status. Verify full operation of the door for proper alignment, speed for closing and lifting safely, and confirming all essential safety systems are in working condition. Perform all repairs to return the door to safe, smooth, and reliable operations. Dispose of all removed and/or replaced materials off-site, and clean work area upon completion of the repairs. 5. Quotes are considered firm for a period of 60 days from the date of the quote submitted. Should C-TRAN seek to go beyond that period, C-TRAN may request in writing for bidders to extend the pricing for an additional 30 days for a total period to not exceed 90 days. 5.1 Pricing C-TRAN is seeking lump sum pricing for the inspection of the bi-fold glass door garage door system and lump sum for materials for returning the garage door system to full operations. Pricing provided must be submitted for this sourcing event using the event's bid form to properly calculate the values for review. Failure to submit the information into the bid form will make a bidder non-responsive for this sourcing event. Bidders may elect to upload their company's quote as an additional document as a supplemental for the work but shall not replace the requirement for completing the bid form with their pricing. 6. Each occurrence Certificates of Insurance and Endorsements acceptable to C-TRAN shall be filed with C-TRAN prior to commencement of the Work. These Certificates and the insurance policies required by this section shall contain a provision that coverages afforded under the policies will not be reduced, canceled or allowed to expire until at least 30 days prior written notice has been given to C-TRAN. C-TRAN shall be included as an additional insured on all such policies. Information concerning reduction of coverage shall be furnished by the Contractor with reasonable promptness in accordance with the Contractor’s information and belief. The Contractor shall furnish one copy of certificates of insurance herein required. The Contractor shall furnish to C-TRAN copies of any endorsements that are subsequently issued amending coverage or limits.
