This is an additional Form 470 to be compliant with E-rate rules for filing under Internal Connections and Basic Maintenance. Please replace this Form 470 and disregard Form 470 #260024331. The Library district is seeking bids for subscriptions and support of already purchased equipment. Please see attached RFP for detailed information. Please price as a 12-mo. term agreement to include voluntary extension options. All quantities cited in this 470 are estimates only. All ineligible services, components, fees, and functionality should be clearly identified and cost allocated. Please direct all questions to the Technical Contact and send all responses to both kpulliam@icc-psd.com and dstone@ppld.org with the 470 number and Library name cited in the subject line. Bids must also include estimated FEES and TAXES and must be received within the 28-day bidding period. SPAM or Robotic responses will not be considered.
