Page 2 of 2 Page 2 of 2 36C24221Q1493 SPECIAL NOTICE NOTICE OF INTENT TO AWARD SOLE SOURCE ZEISS FORUM The Department of Veterans Affairs, Network Contracting Office NC02 in conjunction with the Western New York Health Care System Syracuse VA Medical Center, 800 Irving Avenue, Syracuse, NY 13210, ACQUISITION & MATERIEL MGMT intend to solicit and negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. The NAICS code is 334510 with a small business size standard of 1,250 employees. The VA intends to solicit and negotiate with PRO AIM for ZEISS FORUM SOFTWARE This contract will be a Firm Fixed Price sole source. Zeiss FORUM is a software system that integrates all of our instruments and testing into one location at each of our workstations. It will be used on each patient encounter at the Eye Clinic in both the Optometry and Ophthalmology services. On average, we see between 60-80 patients per day collectively in the Eye Clinic. Syracuse eye clinic is seeking to install and implement the Zeiss FORM Eye Care Picture Archiving and Communication Systems (PACS) solution and Zeiss IOL Master replacement in order to facilitate the care of patients referred amongst facilities within the Syracuse = VA and ensure interface compatibility with the VA s Veterans Health Information and Technology Architecture (Vista)/ Computer Patient Record System (CPRS). The implementation of Zeiss FORM Eye Care Picture Archiving and Communication Systems (PACS) solution and Zeiss IOL Master replacement will permit/ facilitate the care of patients referred amongst facilities within Syracuse VA and ensure interface compatibility with the VA s Veterans Health Information and Technology Architecture (Vista)/ Computer Patient Record System (CPRS). Other persons may identify their interest by responding to this requirement by Friday, September 17th, 2021 at 5pm EDT. Such capabilities information will be used solely for the purpose of determining whether to conduct this procurement on a competitive basis. - All information shall be furnished at no cost or obligation to the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
