POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:

3. SAM UEI number.
4. NAICS (North American Industry Classification System) code 339114 Dental Equipment and Supplies Manufacturing
