The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2020-04 effective 01/15/2020. The associated North American Industrial Classification System (NAICS) code for this procurement is 334517 with a small business size standard of 1,000 employees. The VA Southern Nevada Healthcare System has a requirement for Hologic SmartCurve Breast Stabilization System. This is a solicitation open to Small Business only. All vendors must be properly registered in the System Award Management database (SAM) and Vetbiz.gov. This solicitation allows for vendors to offer BRAND NAME ONLY products. All items shall be new, and not refurbished or recycled. All deliveries must be completed by 30 Days ARO to the following destination: 5406 E. El Campo Grande Ave. Las Vegas NV 89115. Price quotes shall include the Duns Number, VIP Verification (if applicable), Small Business Status, an authorized distributor letter on the manufacturer s letterhead, and a copy of a price list or any other supporting information for the price submitted.
