1. Issue Date: 09/30/2013 Close Date: 10/31/2013 1200 EST 2. Requiring Activity: US Army Medical Research Acquisition Activity (USAMRAA) for the US Army Medical Materiel Development Activity (USAMMDA). 3. Description of Requirement: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. 4. Solicitation Number: W81XWH-13-T-0420 This combined synopsis/solicitation is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-69, 03 September 2013. This combined synopsis/solicitation is a 100% small business set-aside procurement using North American Industry Classification System (NAICS) Code 334419. Small Business size standard for this NAICS code is five hundred (500) employees. Medical Support Systems Project Management Office, US Army Medical Materiel Development Activity (USAMMDA) has an emerging need for a 3D scanning and measurement system (3DSMS). The 3D scanning and measurement system can quickly scan complex or large shapes and delicate or hard-to-move objects for inspection, simulation, digital mock-ups, prototype development and design engineering applications. The system will consist of both hardware (handheld scanner) and software (any and all applications) to transform 3D scan and probe data meshes into accurate surfaced and parametric 3D Computer-Aided Design (CAD) models for engineering, product design, rapid prototyping and manufacturing. All quotations shall satisfy and comply with Minimal Essential Characteristics (MEC's) which are provided in the Statement of Need (SON). All quotations shall provide a matrix that directs a point back to all MEC's. Quotations shall include all costs associated with the Delivery Schedule, Installation, Set-up, Training, and Warranty. Contract shall be Firm Fixed-Price. All quotations shall be evaluated in accordance with FAR 52.212-2. Quotations shall be prepared in accordance with FAR 52.212-1. Additional Instructions to Offerors Due to the difficulties in receipt of mail through normal postal services, quotations shall be sent via e-mail to the POC for this combined synopsis/solicitation, to contact@example.gov . The Offeror is responsible for ensuring receipt by the POC. Questions shall be submitted to the Point of Contact (POC); Roman Laterza at contact@example.gov , no later than 11 October 2013 1200 EST. Follow-up additional questions will not be accepted after this date. Questions and answers will be provided via an amendment to this combined synopsis/solicitation. Quotations are due by 31 October 2013 1200 EST. Quotations shall provide a price for ALL items listed. Your submission should include two separate parts: a Price proposal and a Non-Price proposal. The Non-Price proposal shall be submitted in MS Word or searchable PDF format. The Price proposal shall be submitted in MS Word or MS Excel 2000 or higher, but not greater than MS Word or MS Excel 2003 format. The firewall utilized by the Government contracting office does not authorize or accept zip files. Quotations shall include all delivery charges for products specified in the vendor's quotation.
