Usability and effectiveness of the intervention will be assessed by 1) Veteran completion of the best possible medication list with and without assistance from a coach; 2) total number of PACT Nurse Case Manager reconciliations with and without the assistance of a study coach; 3) concordance between the best possible medication list and the CPRS medication list abstracted from CPRS four weeks after patient generation of the list; and 4) potential for adverse drug events and therapeutic duplication in the final medication list provided by the patient as compared with the pre-intervention medication list.
