Barrels, formaldehyde, Unit formalin for Hatcheries IFB -JF26-86

The State of Utah Division of Purchasing, on behalf of the DNR Division of Wildlife Resources, is soliciting bids under IFB JF26-86 to establish a five-year contract for the supply of 150 55-gallon barrels of therapeutic-grade 37% formaldehyde, FDA approved for use with fish and eggs, for hatchery operations. The contract will be awarded to the lowest cost responsive and responsible vendor, with pricing guaranteed for one year and deliveries required to occur between April 15 and October 1 annually. All shipments must be F.O.B. Destination Freight Prepaid and Allowed, with the contractor bearing all transportation and handling costs, and formaldehyde must not be exposed to temperatures below 40°F during transit. The product must meet recognized labeling standards such as Parasite-S or Formalin-F, and the contractor must comply with all federal and state regulations, including CFR Appendix II to Part 200 if federal funds apply. Vendor eligibility requires certification that they are not engaged in any economic boycott of the State of Israel, are not providing forced labor products or utilizing restricted foreign entities, have never been debarred by any government agency, and are registered in E-Verify for all Utah-based employees. The contractor must adhere to strict packaging, labeling, and delivery specifications, and all items are subject to inspection within thirty days of delivery, with liability remaining with the contractor until final acceptance. Invoices must be submitted within thirty days of delivery, listing the contract number, and payment will be issued within thirty days of receipt of a correct invoice, with interest applicable under the Utah Prompt Payment Act if payment is delayed beyond sixty days. The contractor must maintain records for at least six years post-final payment and grant the State of Utah, federal auditors, and designated personnel unrestricted access to those records. The State retains the right to require background checks for personnel with access to secure facilities and mandates compliance with federal and state employment laws, including Title VI and VII of the Civil Rights Act, the Rehabilitation Act, and the Americans with Disabilities Act. Workers’ compensation and $1 million per occurrence commercial auto liability insurance are required. The contract contains no automatic renewal provisions and any vendor-specific terms or conditions are null and void. Responses must be submitted electronically via U3P by the June 16, 2026 deadline, and failure to meet any mandatory minimum requirement will result in immediate disqualification.
