Full Description

Pursuant to FAR Subpart 13.106-1(b)(1), the Gallup Indian Medical Center, Gallup, NM, intends to pursue a sole source acquisition to Sangre de Cristo Community Health Partnership, 1441 S. Saintfrancis Dr. Ste. A, Santa Fe, NM 87505, to provide Behavioral Health Counseling services and Screening, Brief Intervention and Referral to Treatment (SBIRT) services for the Gallup Indian Medical Center, Gallup, New Mexico. The North American Industry Classification System code is 624190 and the business size standard is $10.0 Million. Only one (1) award will be made as a result of this solicitation. This will be awarded as a firm fixed price Non-Personal Service type contract. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures. The anticipated period of performance is for one (1) year, beginning September 1, 2013 through August 31, 2014. Contractors must be registered in the System for Award Management (SAM) in order to receive a contract award. Contractors may access the SAM website at: https://www.sam.gov/portal/public/SAM/. The purpose of this contract is to: 1) provide a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) to perform in the delivery of patient care consultations, assessments, brief interventions and brief treatments; 2) provide quarterly, two-day trainings sessions in understanding SBIRT and motivational interviewing processes; 3) provide reports outlining patient encounters, services provided and training effectiveness. Personnel performing work under this purchase order shall have all necessary licenses and certification as required by local, State, or Federal Laws and Regulations in order to perform work required by the purchase order. The proposed contract action is for services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. This is not a solicitation for competitive quotes. However, if any interested party, specifically small businesses, believe they can meet the above requirement, they may submit a statement of capabilities within 5 days of publication of this synopsis. All information furnished must be in writing and must contain sufficient detail to determine if it can meet the above unique specifications described herein. Respondents must submit their capability statements via email to contact@example.gov on or before 12:00 PM MST on August 29, 2013 All questions must be in writing and can be emailed. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government.
