This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
This requirement is for one (1) Firm-Fixed-Price (FFP) purchase order for biohazard waste pickup, removal and disposal services to support the Warm Springs Health and Wellness Center (WSH&WC) at the Warm Springs Service Unit (WSSU), Warm Springs, Oregon. The purpose of this requirement is to provide biohazard waste management services to ensure proper handling, removal, transportation, and disposal of biohazard waste while supporting ongoing clinical operations and compliance with applicable Federal, State, Local, Tribal, and IHS regulations and standards (See attachments for details).
PERFORMANCE WORK STATEMENT
BIOHAZARD WASTE PICKUP, REMOVAL, AND DISPOSAL SERVICES
WARM SPRINGS SERVICE UNIT (WSSU)
I. BACKGROUND
The Warm Springs Health and Wellness Center’s mission is to provide an innovative, caring, and diverse healthcare system that is trusted and responsive to the needs of the community. The Warm Springs Health and Wellness Center is a primary care clinic that provides quality healthcare to all eligible American Indians and Alaska Natives in the Warm Springs Service Unit Area. The Warm Springs Health and Wellness Center provides a comprehensive system of healthcare services including medical, dental, optometry, pharmacy, laboratory, radiology, podiatry, and community health services. The Warm Springs Service Unit generates removal, transportation, and disposal of biohazard waste in accordance with applicable Federal, State, Local, Tribal, and IHS requirements.
Due to the ongoing need for proper management and disposal of biohazard waste generated through clinical operation, the Warm Springs Service Unit requires contracted biohazard waste pick up, removal, transportation, and disposal services to ensure safe, timely, and complaint disposal of biohazard waste and support continued healthcare operations.
II. OBJECTIVE
The objective of this contract is to provide safe, timely, and compliant management of biohazard waste generated by the Warm Springs Health and Wellness Center. The contractor shall provide services necessary to support the proper management and disposal of biohazard waste in accordance with applicable Federal, State, Local, Tribal, and IHS requirements.
The contractor shall provide biohazard waste disposal services as coordinated with the Contracting Officer’s Representative (COR).
III. PERIOD OF PERFORMANCE AND LOCATION
A. Location
Services shall be performed at:
Warm Springs Health and Wellness Center

Warm Springs, OR 97761
B. Period of Performance
The period of performance shall be one (1) year base period with four (4) one-year option periods, for a total potential period of performance of five (5) years. The base period shall begin on the date of award. The option periods may be exercised by the Government in accordance with the terms and conditions of the contract.
C. Clinic Schedule
Clinic hours are generally:
- Monday, Tuesday, Thursday, Friday: 8:00 AM – 5:00 PM
- Wednesday: 12:00 PM – 5:00 PM
The contractor shall coordinate the biohazard waste pickup schedule with the Contracting Officer’s Representative (COR). Pickup services shall be performed in accordance with the schedule established by the COR and the contractor to ensure timely removal of biohazard waste.
IV. PERFORMANCE-BASED REQUIREMENTS
A. Performance Objectives
The Contractor shall provide comprehensive performance-based biohazard waste management services that ensure the safe, timely, and compliant handling and disposal of biohazard waste generated by the Warm Springs Health and Wellness Center (WSSU).
The Contractor shall achieve measurable outcomes consistent with applicable Federal, State, Local, Tribal, and Indian Health Service (IHS) requirements and accepted standards for biohazard waste management and disposal.
Performance shall be evaluated based on timely service, proper handling and removal of biohazard waste, compliance with applicable regulatory requirements, proper disposal practices, documentation accuracy, and timely resolution of service issues.
This avoids carrying over the language such as patient access, clinical outcomes, nursing practice, and clinical productivity, which don't apply to the biohazard waste contractor.
B. Performance Standards
The Contractor shall:
- To perform biohazard waste collection services scheduled in a timely and consistent manner;
- Properly handle, collect, transport, and dispose of biohazard waste in accordance with applicable Federal, State, Local, Tribal, and I.H.S. requirements;
- Maintain compliance with applicable environmental, health, safety, and biohazard waste management requirements;
- Use appropriate equipment, containers, vehicles, and handling procedures to prevent spills, leaks, exposure, or contamination;
- Maintain the cleanliness and integrity of waste collection areas and containers during the performance of services;
- Provide required documentation, manifests, disposal records, or other records demonstrating proper collection, transportation, treatment, and disposal of biohazard waste, as applicable;
- Promptly report any spills, damaged containers, missed collections, safety incidents, or other conditions that may affect the proper handling or disposal of biohazard waste;
- Maintain all required permits, licenses, registrations, certifications, and authorizations necessary to perform biohazard waste collection, transportation, treatment, and disposal services throughout the contract period.
Performance deficiencies identified by the Government may require corrective action plans and may impact continuation of services.
V. Tasks
A. General Requirements
The contractor shall provide biohazard waste management and disposal services in a safe, responsible, professional, and timely manner consistent with applicable Federal, State, Local, Tribal, and IHS requirement and accepted standards for biohazard waste management, transportation, and disposal.
The contractor shall:
- Provide for the pickup, removal, transportation, and disposal of biohazard waste generated by the Warm Springs Health and Wellness Center;
- Handle and transport biohazard waste in accordance with applicable Federal, State, Local, Tribal, and IHS requirements;
- Ensure biohazard waste is properly contained, handled, transported, and disposed of in accordance with applicable requirements;
- Provide all necessary labor, equipment, supplies, containers, and transportation required to perform the services;
- Maintain required documentation and records associated with the pickup, transportation, and disposal of biohazard waste;
- Coordinate the waste pickup schedule with the Contracting Officer’s Representative (COR);
- Promptly notify the COR of any issues, spills, damaged containers, missed pickups, or other conditions that may affect the safe and timely removal of biohazard waste;
- Comply with all applicable Federal, State, Local, Tribal, and IHS requirements governing biohazard waste management, transportation, and disposal.
The contractor shall maintain all required documentation related to the pickup, removal, transportation, and disposal of biohazard waste in accordance with applicable Federal, State, Local, Tribal, and IHS requirements and WSSU procedures.
B. Biohazard Waste Disposal Services
1. Biohazard Waste Task Requirements
The Contractor shall provide biohazard waste pickup, removal, transportation, and disposal services in accordance with the requirements of this Performance Work Statement and all applicable Federal, State, Local, Tribal, and IHS requirements.
Biohazard waste disposal responsibilities include, but are not limited to:
- Providing all necessary labor, equipment, supplies, containers, and transportation required to perform the services;
- Picking up biohazard waste from designated collection areas at the Warm Springs Health and Wellness Center;
- Properly handling, removing, transporting, and disposing of biohazard waste in accordance with applicable requirements;
- Maintaining appropriate procedures for the safe handling and transportation of biohazard waste;
- Providing required documentation associated with the pickup, removal, transportation, and disposal of biohazard waste;
- Coordinating the service schedule with the Contracting Officer’s Representative (COR);
- Promptly notifying the COR of missed pickups, spills, damaged containers, or other issues that may affect the safe removal, transportation, or disposal of biohazard waste; and,
- Complying with all applicable Federal, State, Local, Tribal, and IHS requirements governing biohazard waste management, transportation, and disposal.
2. Biohazard Waste Disposal Performance Outcomes
The Contractor shall:
- Provide safe, timely, and reliable biohazard waste pickup, removal, transportation, and disposal services;
- Maintain compliance with all applicable Federal, State, Local, Tribal, and IHS requirements;
- Ensure biohazard waste is properly contained, handled, transported, and disposed of;
- Maintain accurate and timely documentation related to biohazard waste pickup, removal, transportation, and disposal;
- Complete scheduled pickups as coordinated with the COR;
- Promptly report missed pickups, spills, damaged containers, or other service-related issues to the COR;
- Maintain professional conduct and safe work practices while performing services at the Warm Springs Health and Wellness Center.
C. Service Coordination
The Contractor shall coordinate biohazard waste pickup and disposal services with the Contracting Officer’s Representative (COR).
The Contractor shall notify the COR of any issues that may affect scheduled services, including missed pickups, access issues, damaged or improperly contained waste, spills, or other conditions requiring attention.
E. Recordkeeping and Documentation
The Contractor shall:
- Maintain accurate, timely, and legible documentation related to biohazard waste services;
- Maintain records of biohazard waste pickups, removal, transportation, and disposal;
- Maintain required waste disposal manifests and related service documentation;
- Provide documentation to the COR upon request;
- Comply with all applicable Federal, State, Local, Tribal, and IHS requirements for biohazard waste documentation and recordkeeping.
VI. GOVERNMENT RESPONSIBILITIES
- Provide designated areas for biohazard waste collection and storage.
- Provide reasonable access to designated waste collection areas for the Contractor to perform scheduled services.
- Provide the contractor with applicable site-specific procedures for biohazard waste handling and removal.
- Coordinate with the contractor regarding the established biohazard waste pickup schedule.
- Notify the contractor of clinic closures or emergencies that may affect scheduled services as soon as practicable.
Equipment available onsite includes designated biohazard waste collection and storage areas and containers provided by the Warm Springs Service Unit, as applicable.
VII. QUALIFICATIONS
A. Biohazard Waste Disposal Contractor Qualifications
The Biohazard Waste Disposal Contractor shall possess:
- All applicable licenses, permits, registrations, and certifications required to perform biohazard waste pickup, transportation, and disposal services
- Demonstrated experience providing biohazard waste management and disposal services;
- Compliance with all applicable Federal, State, Local, Tribal, and IHS requirements governing biohazard waste management, transportation, and disposal;
- Qualified personnel, equipment, vehicles, and resources necessary to safely perform the required services;
- Current documentation demonstrating compliance with applicable regulatory and disposal requirements
B. Contractor Qualifications
The Biohazard Waste Disposal Contractor shall possess:
- All applicable licenses, permits, registrations, and certifications are required to perform biohazard waste pickup, transportation, and disposal services;
- Qualified personnel with appropriate training and experience in biohazard waste handling and disposal;
- Appropriate vehicles, equipment, containers, and other resources necessary to safely perform the required services;
- Demonstrated experience providing biohazard waste management and disposal services;
- Compliance with all applicable Federal, State, Local, Tribal, and IHS requirements governing biohazard waste management, transportation, and disposal.
C. Required Documentation
The Contractor shall complete and submit all required documentation including:
- Applicable licenses, permits, registrations, and certifications;
- Documentation of required biohazard waste handling, transportation, and disposal training;
- Documentation demonstrating compliance with applicable Federal, State, Local, Tribal, and IHS requirements;
- Required waste disposal manifests, certificates, or other disposal documentation;

VIII. DELIVERABLES, QUALITY ASSURANCE, AND PERFORMANCE METRICS
A. Deliverables
The Contractor shall provide:
- Scheduled biohazard waste pickup, removal, transportation, and disposal services;
- All necessary labor, tools, equipment, supplies, containers, and transportation required to perform the services;
- Proper handling, transportation, and disposal of biohazard waste in accordance with applicable requirements;
- Timely completion of scheduled services;
- Documentation related to biohazard waste pickup, removal, transportation, and disposal, as required;
- Compliance with applicable Federal, State, Local, Tribal, and IHS requirements;
- Prompt notification to the COR of any circumstances that may prevent or delay a scheduled biohazard waste pickup.
B. Performance Metrics
The Government may evaluate contractor performance using the following indicators:
Performance standards include:
- Scheduled Pickup Services: Completion of scheduled biohazard waste pickups within the required timeframes;
- Service Reliability: Contractor communicates promptly with the COR when unable to complete a scheduled pickup;
- Waste Handling and Disposal: Biohazard waste is properly handled, removed, transported, and disposed of in accordance with applicable requirements;
- Regulatory Compliance: Compliance with applicable Federal, State, Local, Tribal, and IHS requirements;
- Documentation: Required biohazard waste service documentation is accurate and completed within required timeframes;
- Professional Conduct: Contractor personnel maintain professional conduct while performing services at the Warm Springs Health and Wellness Center;
- Service Issues: Timely notification and appropriate resolution of issues affecting biohazard waste pickup, removal, transportation or disposal services.
C. Quality Assurance Surveillance
The Contracting Officer Representative (COR) may monitor performance through:
- Review of scheduled pickup completion;
- Review of biohazard waste pickup, removal, transportation, and disposal services;
- Review of required documentation;
- Monitoring schedule adherence;
- Review of communication regarding missed or delayed pickups;
- Review of compliance with applicable Federal, State, Local, Tribal, and IHS requirements.
Failure to meet the established performance standards may require corrective action and may result in contract remedies consistent with applicable Federal acquisition regulations.

The Contracting Officer Representative (COR) shall coordinate service scheduling, technical oversight, and review of Contractor performance.
The COR does not have authority to modify contract terms or funding.
X. PAYMENT SCHEDULE
A. Rates
The firm-fixed price awarded shall be the accepted price and shall include all costs associated with providing the required biohazard waste pickup, removal, transportation, and disposal services.
B. Payment
Payment shall be made only for services rendered and accepted by the Government.
Payment shall comply with the Prompt Payment Act and applicable Federal regulations.
XI. INVOICING
Invoices shall be submitted electronically through the Invoice Processing Platform (IPP) in accordance with applicable Indian Health Service (IHS) payment procedures and the terms of the purchase order. The Contractor shall ensure that all invoices contain the required information to facilitate timely processing and payment.
XII. ADDITIONAL TERMS AND CONDITIONS
A. Contractor Compliance and Qualifications
The Contractor shall maintain all applicable licenses, permits, registrations, certifications, and other qualifications required to perform the biohazard waste services throughout the period of performance. Failure to maintain required qualifications or comply with applicable requirements may result in contract remedies consistent with applicable Federal Acquisition Regulations.
B. Government Furnished Equipment
The Government does not anticipate the need for Government-furnished equipment.
The Contractor shall provide all necessary tools, equipment, supplies, containers, transportation, and other resources required to perform the biohazard waste pickup, removal, transportation, and disposal services.
C. Privacy and Security Requirements
The Contractor shall comply with:
- All applicable Federal, State, Local, Tribal, and IHS requirements governing biohazard waste management, handling, transportation, and disposal;
- Applicable safety and environmental requirements;
- All applicable requirements for the protection of Government property and information encountered while performing the services.;

Submission of Quotes
Quotes shall be submitted electronically via email to PORAOAcquisition@ihs.gov. The subject line shall include the solicitation number 75H71326Q00033 and the Respondent’s entity name. AFFORDABILITY: The Government seeks quotes with a total evaluated price of less than $19,000.00, including all options. Quoters should not interpret this figure as a ceiling or a floor on quotations. Rather, this information is provided as a courtesy to prospective quoters to assist them in preparing their submissions. The Government may select a quotation for award with a price below, at, or above $19,000.00 if the Government determines that the quotation provides the best value to the Government and sufficient funds are available.

BASIS FOR AWARD
The Government intends to award a contract to the respondent responsible whose quote represents the best value to the Government, considering price and non-price factors. The Government may award other than the lowest-priced Respondent. This procurement is time-sensitive, and the Government will consider the Respondent’s ability to provide timely, reliable, and compliant biohazard waste pickup, removal, transportation, and disposal services to ensure continuity of biohazard waste management services.
The Government reserves the right to:
- Make no award
- Evaluate quotes without discussions
- Communicate with Respondents for clarification purposes only
- Exclude quotes that fail to follow solicitation instructions
EVALUATION FACTORS
Factor 1 – Relevant Past Performance
Respondents may submit up to three (3) recent relevant Past Performance projects. Each example shall include:
- Customer name, phone number, and email
- Contract number and dollar value
- Description of work performed
- Period of performance
Factor 2 – Price
The Government will evaluate the total evaluated price based on the CLIN structure provided (Attachment No. 1 – Pricing Schedule). The price quote shall include all costs necessary to fulfill the requirement, including labor, equipment, transportation, disposal, and all incidental costs required to successfully perform biohazard waste pickup, removal, and disposal services under the contract.
EVALUATION PROCESS
The Government may use a comparative evaluation in accordance with FAR 12.203(c)(2), Evaluation.
