The Centers for Disease Control and Prevention is awarding a sole source firm fixed price purchase order to The Children’s Hospital of Philadelphia under FAR 13.106-1(b) to provide expert neurologic review and classification of patients under investigation for Acute Flaccid Myelitis using the Ambra image cloud platform. The neurologist will analyze MRI images, patient summary forms, neurology consult notes, and related medical data to classify cases as confirmed, probable, suspect, or not a case based on standardized criteria approved by the Council of State and Territorial Epidemiologists. Each classification must be documented using an MRI abstraction and classification worksheet and completed within seven business days of assignment. The neurologist will also participate in monthly AFM case review calls, deliver one presentation during a Clinician Outreach and Communication Activity call to enhance clinical awareness of AFM diagnosis and reporting, and contribute to projects defining imaging characteristics of confirmed AFM cases. All work must be performed remotely from the contractor’s facility in Philadelphia, with performance spanning a base year from May 29, 2026, through May 28, 2027, and two optional one-year extensions. Payments will be made quarterly in four equal installments. The contractor is required to comply fully with HHS and CDC information security and privacy policies including NIST SP 800-171, FIPS 140-2 encrypted handling of Personally Identifiable Information, and Executive Order 13556 for Controlled Unclassified Information. All information and communication technology used must conform to the Revised Section 508 Standards outlined in 36 C.F.R. 1194.1 and Appendices A, B, and C, with remediation at the contractor’s expense if noncompliant. All software developed under this contract must be open source and made available for government-wide and public use, with source code maintained in an approved CDC repository. The contractor must ensure the highest standards of data quality including accuracy, completeness, timeliness, and privacy across all deliverables and must report security incidents within one hour of discovery. Upon contract conclusion, the contractor must sanitize all government information and devices and submit a certification of sanitization and, if applicable, a decommission plan. The government retains full rights to inspect, investigate, and conduct forensic analysis of contractor activities and systems.

Full Description
The Centers for Disease Control and Prevention intends to award a sole source firm fixed price purchase order to The Children’s Hospital of Philadelphia for MRI images along with patient summary forms, MRI reports, neurology consult notes, and other medical information will be reviewed by the neurologist using the Ambra image cloud platform. Based upon the image review, the neurologist determines whether the patients under investigation (PUIs) are consistent with AFM case definitions. Case definitions shall be used by the neurologist to classify PUIs as “confirmed,” “probable,” “suspect,” or “not a case” for surveillance purposes. Case definitions are approved by the CSTE to standardize the collection of data for specific diseases and are updated as appropriate. As part of the classification review process, the neurologist shall complete an MRI abstraction and classification worksheet for each report reviewed in Ambra. In the event of an outbreak, CDC will host a Clinician Outreach and Communication Activity (COCA) Call and the neurologist will present during calls. The calls are intended to improve clinicians’ AFM awareness, detection, and reporting. During the call, clinicians will learn about AFM clinical presentation, diagnosis, and clinical management. The call will also cover the importance of specimen collection and reporting suspected cases to health departments.
The neurologist may also participate in projects related to defining specific imaging characteristics of confirmed cases of AFM, reviewing complete clinical information to provide diagnostic categorization of cases, and other neurologic related projects as identified within the Division of Viral Diseases.

This notice of intent is not a request for competitive quotations; however, all quotations/responses received within insert number of days of the issuance of this notice shall be considered by the government. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government.
