Centralized billing is mandatory, requiring consolidated invoices to be submitted within 90 calendar days of discharge or outpatient encounter using completed CMS-1500 and UB-04 forms, capped at fifty forms per invoice, with corrections resubmitted within 30 days of rejection. All personnel must pass security clearance investigations, and contractors must maintain medical liability insurance at community standard levels.
