COMMUNITY HEALTH WORKER PROGRAM EVALUATION CONSULTANT SERVICES

The County of Riverside, on behalf of Riverside University Health System – Public Health, Injury Prevention Services, is seeking qualified consultants to conduct a comprehensive evaluation of Community Health Worker (CHW) services across Riverside County. This initiative, funded through federal award 5 NE11OE000021-04-00 from the CDC’s Collaboration with Academia to Strengthen Public Health program, aims to assess the effectiveness, outcomes, and population health impact of CHW activities integrated into outreach, chronic disease prevention, injury prevention education, health navigation, and social determinant of health programs. The evaluation will examine how CHWs contribute to community engagement, improved access to care, and better health outcomes among priority populations, with findings intended to strengthen program infrastructure, enhance coordination, improve reporting, and support long-term sustainability. The project has a maximum budget of $75,000 with an anticipated performance period from August 1, 2026, through November 30, 2026, and an option to extend through June 30, 2027. Consultants must work closely with RUHS-PH staff including the Epidemiologist, clinic administration, and program coordinators to conduct mixed-methods analysis using both quantitative and qualitative data, ensuring data cleaning, framework development, and the creation of actionable recommendations. Proposals must be submitted via the Public Purchase website by July 7, 2026, and are subject to strict compliance requirements under California’s Mini-Brooks Act, with legal authority to operate in California and, for non-profits, good standing with the Department of Justice serving as mandatory pass/fail gates. Bidders must provide fully inclusive blended rates covering all expenses, including travel, with no reimbursement for unlisted costs. Required deliverables include a detailed evaluation plan, monthly summary reports due by the 5th of each month featuring progress tracking, data collection status, stakeholder engagement, and planned activities, as well as a finalized unified results summary. All contractors must undergo DOJ criminal background checks on personnel, comply with U.S. and California economic sanctions related to Russia and Iran, maintain appropriate insurance, and submit financial statements no older than twelve months. Proposals must adhere precisely to formatting guidelines, including labeled sections, separate financial files, and signed cover pages, with non-compliance resulting in disqualification. The evaluation will serve as a critical tool to inform strategic planning, guide continuous quality improvement, and strengthen future funding applications for CHW services in the region.
