Full Description

NYC Care (the Program) is a health care access service that provides primary, preventive, and specialty care at NYC Health + Hospitals locations, to include NYC Health + Hospitals major hospitals and its Gotham Health Centers, as well as other authorized facilities. Eligibility is open to New York City residents, regardless of immigration status, who are ineligible for health insurance or who cannot afford insurance. The healthcare available through NYC Care will be priced on a sliding scale (which may vary across locations), to ensure affordability. NYC Care, through its participating facilities, provides a primary care provider, access to specialty care, behavioral health services, prescription drug benefits, as well as 24/7 customer service to seamlessly connect patients to healthcare.

After launching in August 2019, NYC Care currently has an enrollment of approximately 125,000 patients, and the Program reflects an investment by the City of at least $100 million annually in care provided to New Yorkers. This investment has been paying off. Our research shows that among members new to primary care, NYC Care achieved a high primary and specialty care engagement rate, with even higher engagement when looking at members who had two or more chronic diseases and spoke English as a second language (ESL).

In addition to supporting patient care, NYC Care has funded a community grant program to engage New York City nonprofit organizations to drive NYC Care enrollment and outreach. This new RFP reflects how this grant program is evolving to further establish NYC Care as a bright spot for New Yorkers, regardless of immigration status, in the ever-changing landscape of health care coverage and access in New York City.

NYC Care seeks to contract with experienced 501c3 community-based organizations to achieve the goals of enrolling eligible New Yorkers into NYC Care, regardless of immigration status, as well as conducting NYC Care education and outreach activities in TRIE zip codes that promote NYC Cares access to quality and affordable health care. To reach this goal, this grant program is seeking organizations to apply for a 3-year contract (July 1, 2027 to June 30, 2030) in one of two Grant Categories (Proposer cannot apply for both categories):

            (1) NYC Care Direct Enrollment & Outreach Partner:  Primary responsibilities are to:

  • screen applicants through the New York State of Health (NYSoH) Marketplace and facilitate Direct Enrollment for applicants who are eligible for NYC Care as new or renewing members;
  • connect eligible applicants with special circumstances to NYC Health + Hospitals Financial Counselors; and
  • engage New Yorkers through outreach activities in select NYC TRIE Zip Codes, informing and educating them about NYC Care.

(2)  NYC Care Outreach & Referral Partner: Primary responsibilities are to:

  • engage New Yorkers through in-person and digital outreach activities in select NYC TRIE Zip Codes, informing and educating them about NYC Care;
  • refer eligible New Yorkers to make appointments with NYC Care Authorized Enrollers for enrollment as new or renewing members; and
  • promote primary & specialty care access (in-person and/or telehealth), as well as other special health initiatives that may be targeted to NYC Care members through in-person and digital campaigns.
