Explore Academy - Rio Rancho is seeking proposals on the following Fiber Extension bid request:
1. Running of fiber from 7000 Zenith to IT Network MDF closet located at 6090 Zenith approximately 250 linear feet from building to building in 4 PVC Schedule 80 Conduit. Optical Fiber to be OM4 MM 12 strands - to include innerduct. Fiber needs to be extended through building and terminated to main server room as indicated on Exhibit B. Pathway to be determined in field in coordination with Explore. Bid should include estimate for run length. 2. Running of fiber from 6085 Zenith to IT Network MDF closet located at 6090 Zenith approximately 215 linear feet from building to building in 4 PVC Schedule 80 Conduit. Optical Fiber to be OM4 MM 12 strands - to include innerduct. Fiber needs to be extended through building and terminated to main server room as indicated on Exhibit B. Pathway to be determined in field in coordination with Explore. Bid should include estimate for run length.
*Price needs to include traffic control plan, boring, permitting, terminations, testing, and certification reports. Reports to be issued to owner.

Although not a mandatory requirement, service providers should indicate if they are willing to invoice USAC directly, providing the school with the applicable discounts on their monthly billing. The email subject of your response should be titled with the schools name and Form 470 # for which you are bidding. The school, at their discretion, may reject responses sent to the incorrect email address and/or with the incorrect email subject . All ineligible charges must be clearly identified and cost allocated. If applicable, also list any taxes, fees, or surcharges. Any bids that do not address the actual services or equipment requested on this form will be automatically disqualified.
