The School Board of Polk County, Florida (hereafter referred to as the “Board” or the “District”) is seeking proposals from experienced, qualified proposers (hereafter referred to as ‘bidder’ or ‘vendor’) to establish a contract for self-funded health plan administrative services effective January 1, 2025.
The purpose of this RFP is to offer experienced and qualified firms an opportunity to submit written proposals for the District’s consideration. This procurement may result in a multiple vendor award, however only one vendor will be selected for each option requested below:
• Medical Health Plan Administrative Services Only (ASO) • Pharmacy Benefit Manager (PBM) services • Health Management/Concierge Care Coordination Services • A fully integrated solution that includes the services described above.
The District is seeking options for consideration with the goal to provide a health plan that will:
