*In order to enter and perform services in Federal Facilities, all members of the crew/contractor shall have a valid
REAL ID*
The NYARNG has a requirement for 3 dedicated ALS Ambulance with Crew (6 EMT certified Personnel) The ALS
Ambulance Crew must consist of one (1) National Certified Paramedic and one (1) National Certified EMT per
ambulance for a total of three (3) Nationally Certified Paramedics and three (3) Nationally Certified EMTs for this
requirement. Two Ambulances as primaries and one in reserve to Urban Search and Emergency Response Facility
Training Area at 1 Garrison Road, Annville, PA 17038, Fort Indiantown Gap on 29MAY-31MAY from 0700-1900 hrs.
In the event either of the primary ALS Ambulances and crew leave for any reason the reserve ALS ambulance and
crew must be dispatched prior to the 1st crew leaving. **** See attached RFQ and PWS for a more detailed
description of this requirement.****
All quotes must be in accordance with the Performance Work Statement (PWS). The PWS is incorporated in the
attached RFQ. Basis of award is the lowest price that conforms to the requirement of the PWS. Wage Rate
Determination for Lebanon County, PA applies and is incorporated in the attached RFQ. This requirement is a small
business set-aside. Offerers must have Active registration and no exclusions in SAM.gov
All quotes must be submitted vis email to Cody Anna at cody.m.anna.civ@army.mil and Rick Powell at richard.w.powell44.civ@army.mil** All amendments will be posted to www.sam.gov, it is the vendor's responsibility to
check if any amendments have been issued at least 24 hours prior to the closing date of the solicitation. Individual
