The work is structured as a subcontract under the Administration for Community Living, part of the Department of Health and Human Services, and is aligned with the NAICS code 813910, indicating it involves civic and social organizations engaged in advocacy and support services. The goal is to ensure the continuity and expansion of critical dementia care programs by identifying, applying for, and managing external grants and other financial resources that will sustain operations into the future. The effort requires a deep understanding of funding streams relevant to aging and community-based health services, as well as the ability to translate program needs into compelling, compliant grant proposals. Sustainability planning involves assessing internal capacities, building partnerships, diversifying revenue sources, and creating measurable outcomes to demonstrate impact to funders. The work is critical to maintaining access to care for vulnerable populations and must be executed with precision to meet the agency’s mission of supporting community living and independence for older adults and individuals with disabilities.
