FEDBIZOPPS Special Notice VA-248-11-RQ-0575 The Department of Veterans Affairs, West Palm Beach Medical Center intends to procure on a sole source basis with the small business company, Dee Maria Plumbing , Inc. at 4601 Georgia Ave, West Palm Beach FL 334075-2811. This procurement is for a service contract to provide all labor, tools, materials and special equipment needed to perform annual testing, certifying, maintenance and repair of the facility's back flow preventers. The contractor shall provide all labor, tools, materials and special equipment to provide annual testing and certification of facility back flow preventers. The method of procurement shall be a firm fixed price contract for services stated herein. The North American Industry Classification System (NAICS) code shall be 332913. The period of performance for the contract base year shall be June 1, 2011 - 05/31/2012 with four one-year option periods. All testing results and reports related to said equipment is required by July 1st each year. The contractor is required to be certified with Rivera Beach, Florida as an authorized Back Flow Preventer Testing/Certifying business. It is imperative that VA procure this service contract with a contractor located within the Florida area in the event or to prevent emergencies. The proposed contract action is for services and the Government intends to procure with only one source under the authority of FAR 6.302 and FAR 15.308. This notice of intent is not a request for quotations. Interested parties that believe they can meet these needs must supply information about their capabilities to the point of contact, along with supporting evidence to the no later than 12:00PM EST, 05/25/2011. All information shall be furnished at no cost or obligation to the Government. This award will be made using commercial acquisition procedures in accordance with FAR PART 12. The anticipated award date is May 31,2011. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government.
