The Department of Veterans Affairs, Network Contracting Office (NCO) 6 on behalf of the Fayetteville VAMC is conducting Market Research through this sources sought notice to help determine the availability of qualified Businesses capable of providing window replacement services at the Dental Department. The Contractor shall complete the work required to replace the broken glass panels of the windows in the Dental Department in Building#1 at the Fayetteville VA Medical center. It is highly advisable for the contractor to conduct a site visit in order to gather all pertinent information regarding this requirement. The contractor shall furnish labor, parts, supplies and equipment necessary to complete the requirements. Services should be performed in accordance with terms and conditions, in this statement of work and be in compliance with all manufacturers technical specifications. The contractor will provide all material, labor, and tooling to execute the job. This SOW requires the work to be performed by qualified and certified Contract Professionals. The Contracting Officer or COR reserve the right to request authentication of factory training certificates or credentials from the Contractor at any time for any personnel who are servicing facility equipment. Upon completion of work the Contractor shall leave a copy of the service report with the using service and submit to the same location as designated above a service report annotating what service, calibration, etc., were performed and whether the work was done. The service designee and the FSE (Field Service Engineer) attesting that the completed work is in satisfactory working order and is functioning at an acceptable level of performance shall sign this report. The contractor shall sign out at the designated location before leaving. Failure to furnish a service report or obtain a signed copy of the service report could result in delay of payment or non-payment. The COR or Alternate COR in the using service has the authority to approve/request a service call from the contractor. The Contractor shall immediately notify the COR of any deficiencies, which are a hazard. It shall be the responsibility of the FVAMC to take the necessary actions to initiate any Interim Life Safety Measures (ILSM).

All respondents shall indicate/provide the following: Whether or not your company has an active SAM registration and the companies Unique Entity Identifier. A copy of its capabilities statement. Business Size Failure to provide the above information may result in the potential offeror being considered incapable of meeting the VA s needs. The capability statements received from this market research are for planning purposes only and will assist the Government in planning its acquisition strategy. This is strictly market research, and the Government will not entertain questions concerning this market research. The Government will not assume any costs incurred by the contractor in the preparation of responses.
