REQUESTS FOR PROPOSALSA. Purpose and BackgroundPURPOSE: The City is seeking to establish one (1) or more contracts with qualified medical providers for post-offer and periodic medical examinations/medical surveillance for current and future employees. This document constitutes an RFP from qualified individuals, firms, or organizations to provide health and medical services as described herein. The purpose of this RFP is to conduct a competitive process to create a contract(s) from which the City may acquire these services. When preparing a Proposal, Contractors should specifically identify their capability to:1. Have a minimum of one (1) worksite located in Flagstaff, Arizona, with standard business hours and a minimum four (4) day per week service.2. Provide the personnel necessary to provide the services contained within the Scope of Work.3. Provide related written reports to the City within three (3) business days of service or of request by the City.4. Provide a reference for two (2) current contracts where post-offer and periodic medical examinations/medical surveillance are a component of that contract.BACKGROUND: The City has a need for various medical examinations for current and future employees. The term “employee” may substitute for the term “candidate” or “work injury personnel” depending on the purpose of the service to be provided.The City may have ongoing requirements for coordinating and reviewing medical physical exams for a variety of positions that may put employees at risk of occupational illness or injury. Every exam should begin with a general Health History Questionnaire and Physical Examination form. Additional specific components, like a pulmonary function test (PFT), immunization or vaccination, lift test, etc., may be added depending on the job requirements. When United States Department of Transportation (USDOT) and Arizona Peace Officer Standard Training (AZPOST) exams for initial hiring are required, Contractor shall perform those exams in conjunction with the General Exam referenced below. Other specific tests and exams may be ordered independent of the General Exams.B. Cost ProposalContractor must use the Fee Schedule provided by the City in the RFP document Attachment 1. Failure to use the RFP fee schedule provided by the City shall be cause for rejection of a Proposal. All fees shall include all labor, materials, and equipment needed to perform the required services as well as all charges, fees, and related expenses to perform the required work. No separate or additional costs will be paid by the City except for bona fide emergency situations.
