The Utah Department of Health and Human Services is awarding a sole-source contract to Problems Anonymous Action Group Incorporated for comprehensive wrap-around case management, drop-in services, and permanent housing support for individuals with serious mental illness who are homeless or at risk of homelessness, specifically within Weber County. The contractor is responsible for establishing an internal quality management system, submitting quarterly and annual performance reports, and ensuring compliance with federal and state privacy standards including 42 C.F.R. Part 2 and 2 CFR Part 200. Financial viability must be sustained for twelve months after each fiscal year, and records including client data and financial documentation must be retained until June 30, 2030, with strict confidentiality obligations continuing through that date.
